Why I Created Assess Treat Record

Assess Treat Record was created from a real problem I experienced in clinical practice: I wanted a faster, more structured way to complete case notes without spending unnecessary time typing after treating clients.

The problem I wanted to solve

Use Your Hands to Treat — Not Type

As a remedial massage therapist, my hands are used throughout the working day to assess and treat clients. After a full day of hands-on treatment, I didn't want to spend unnecessary additional time typing lengthy clinical notes.

I also found that many systems gave therapists somewhere to write their notes, but they didn't necessarily follow the way I assessed, treated and reassessed my clients.

That Idea Became Assess Treat Record

I wanted clinical documentation to support the treatment process rather than feel like a separate administrative task.

Assess Treat Record was developed to bring assessment, treatment, evaluation, aftercare and future treatment planning together in one structured workflow designed around remedial massage practice.

A better clinical workflow

I Wanted the Case Note to Follow the Treatment

Rather than starting each consultation with a blank page, I wanted a system that connected the different parts of the clinical process.

Assess → Treat → Record → Reassess → Plan

    Record the client's presentation and relevant clinical assessment.

  2. Treat

    Document the areas, muscles and treatment techniques relevant to the consultation.

  3. Record

    Keep the clinical information structured within the client's record.

  4. Reassess

    Record relevant changes following treatment.

  5. Plan

    Document aftercare and what should be reviewed or treated at the next consultation.

Technology Should Support Clinical Reasoning — Not Replace It

Assess Treat Record is designed to support the therapist's own assessment, documentation and clinical reasoning. The therapist remains responsible for the clinical decisions and information recorded for each client.
